Nursing services provided at no charge include: screening, evaluation/assessment, service coordination and development of an individualized family service plan.

Referral criteria: Any child from birth to age 3 with an established medical condition or risk of developmental delays.

How do I enroll in the Early On Program?
Simply call 1-800-EARLY ON.

What will Early On do for me?
The Early On nurses will visit your home and complete a developmental assessment of your child. From this assessment, a family service plan will be developed which will include coordination of medical care and referral to appropriate resources.

What other services can Early On provide?
Early On provides assistance/referral in the following areas for children based on their needs:

  • Physical therapy
  • Occupational therapy/assistive technology
  • Speech therapy
  • Hearing testing/audiological services
  • Vision testing/vision services
  • Psychological/counseling services
  • Social work services
  • Nutrition counseling
  • Parenting education
  • Medical services for evaluation and diagnosis
  • Nursing services
  • Transportation to medical appointments
